Infants, Children, and Adolescents 1 month to 16 years weighing 6 to 29 kg 1 mg/kg/dose IV twice daily initially; may increase by 2 mg/kg/day at weekly intervals based on response and tolerability. The recommended maintenance dose is 3 to 6 mg/kg/dose IV twice daily.
